Our Solution

We know that patient behaviors are the most critical determinant of health.  Patient behaviors are also the most costly determinant of health care. We also know that patient behaviors happen at home, work and play. Successful chronic care management involves extending the experience and guidance from the clinician’s visit into the daily lives of patients.  Health happens at home; health care support should happen at home, too.

Quvium is focused on providing personalized support to help patients adhere to care protocols each and every day.  We have developed an innovative care management platform for people living with chronic conditions that make it easier for them to do the things that keep them healthy.

The Quvium platform is not disease specific.  It houses 4 analytical components: a predictive health model, a behavioral model, a care community model and an incentives and feedback model. These components can be customized to address different diseases. This platform – Quvium’s IP – can be applied to 50 different chronic diseases and 20 treatment categories. The technology has the potential to dramatically improve care protocol adherence in ‘real-life settings’, and reduce acute care costs.
